Informed Consent Form

This is a legal document and this document is required to have according to Department of Health.

•This document gives you right to perform the service on your client.

•The informed consent form needs to have Name, Age, DOB, Address.

•This form will also contain a section where the artist can write which pigments and blades are used during the procedure.

•Cannot perform lip tattoo on minors under 15. Can perform on 16, 17 year old. But the parent needs to be present and a special form from Department of Health needs to be signed and notarized. ( This depends on the State. Please check the regulations with your state).
